Sarasota County Health Safety Net System of Care Collaboration

This collaboration continues to bring local healthcare providers together to address the challenges of providing health care for uninsured and underinsured individuals in Sarasota County, Florida.

Self-Management Alliance

Managing one’s own healthcare, especially multiple chronic conditions, is essential in order to stem increasing healthcare costs in America. The Patterson Foundation participated in the Self-Management Alliance, a tri-sector collaboration created by the National Council on Aging that explored how self-managed care might be approached to improve health.

SPIRE CoLab

The SPIRE CoLab was a partnership between philanthropy and journalism to inspire communities to take action through solutions journalism, which focuses on responses to social issues.

Suncoast Nursing Action Coalition

The Suncoast Nursing Action Coalition (SNAC) was a regional coalition made up of representatives from major healthcare organizations, schools educating nurses, and community members in Florida’s Charlotte, DeSoto, Manatee, and Sarasota counties to positively influence the health and healthcare outcomes in the nation. The grassroots group worked with the Nursing Action Coalition of Florida to implement changes recommended by the 2010 Institute of Medicine report entitled The Future of Nursing: Leading Change, Advancing Health while addressing the 2020 recommendations from Population Health and the Future of Nursing.

The Bay

The Bay is a signature public park along Sarasota Bay. Designed to be “One Park for All,” The Bay is open and accessible, free and welcoming to the full and rich diversity of the community, now and for generations to come. The Patterson Foundation strengthened The Bay’s evolution through catalytic funding, thought partnership and technical assistance, and challenge match opportunities to incentivize giving from private philanthropy.

The Giving Partner

The Giving Partner helps people make more informed decisions about giving based on in-depth information provided by nonprofits. Together with the Community Foundation of Sarasota County and other funders, The Patterson Foundation supported bringing this unique tool to our region in Florida.

UPLIFT Florida Network

UPLIFT Florida Network is a hub organization created to connect people, organizations, and communities in Florida so we can all thrive in community. The Patterson Foundation strengthened this effort by investing in consulting support to provide facilitation and strategic guidance for the creation and operation of UPLIFT Florida Network and its villages, which are community-based organizations characterized by neighbors helping neighbors.

Veterans Legacy Summit

The Patterson Foundation shared the unique public-private partnership story of Southwest Florida's Patriot Plaza with dignitaries from across the country in November 2014 during the Veterans Legacy Summit. Leaders from the government, military, philanthropy, national veterans-serving organizations, and others supporting veterans came together to share ideas, learn about unique partnerships such as Patriot Plaza, challenge assumptions, and push the boundaries of potential to honor our nation's heroes.
